A LATE REGISTRATION FEE OF $5.00 WILL BE ACCESSED FOR REGISTRATIONS RECEIVED OR POSTMARKED AFTER JULY 15TH. Please include your child(ren)'s name(s) on memo part of check.

REGISTER IN-PERSON

Registrations will be held at the Rec Field gazebo. In case of rain, registrations will be held at the Hubbardston Center School.

Please print one of the above forms and bring it with you along with payment to registration.

FEES

Fall 2009 registration fees are the following:

• $40 for 1 child (all levels)
• $60 for 2 siblings (all levels)
• $75 for 3 or more siblings (all levels)

* Financial aid is available for eligible families (call Patti Hall at 978-928-1268 for details)

LATE FEE

All registrations not postmarked by or received by Wednesday, July 15th will be subject to a $5 late fee.
